Pricing

Initial consultation: $249 $149 (Save $100 if sign-in by March 31,2025.)

• Includes a comprehensive evaluation with a physician, personalized treatment plan, and initial prescription.

Monthly program subscription: $149/month

• Prescription management, ongoing care (incl. one follow-up consultation monthly), adjustments to your plan, and support.

ADD-ON: Medication prior authorization: $150

• This is an additional fee for obtaining insurance approval for your prescribed medication, if needed. 

(Superbill for insurance reimbursement is NOT provided.)

Initial Consultation (30-45min): $350
Each Follow-up Appointment (15-20min): $200
Prior Authorization: $150

Any additional services are charged at our hourly rate ($400)
in 15 min increments.

Additional notes:
Medication prescription is provided during appointments ONLY.
Missed appointments (no-shows) and cancellations within one business day of your appointment incur a $75 fee.
Superbill for insurance reimbursement is provided per request.
